Position Summary: 

Incumbent is responsible for high level assessment, planning and transitions of care for patients with acute and/or chronic psychiatric conditions.


Salary Range: 


Rate    Minimum  $111,373.92    Maximum  $135,768


Required Qualifications:

Ability to establish and maintain effective working relationships across the HealthSystem

Ability to facilitate and lead interdisciplinary rounds

Ability to maintain a work pace appropriate to the workload

All external candidates must have a Bachelor of Science in Nursing

Current CA RN license

Demonstrate experience developing and executing treatment plans for patients

Demonstrated experience performing discharge planning in a hospital or long term medical care facility

Excellent written and verbal communication skills in English

Experience with and understanding of Utilization Review criteria and discharge planning

Familiarity of Joint Commission, CMS, CDPH and CCS requirements

Knowledge of and ability to work within a complex health care system

Minimum five years of clinical / direct patient care experience OR minimum 1 year experience working  as a RN Case Manager

Must demonstrate customer service skills appropriate to the job

Must possess complete understanding of acuity levels for specific patient population

Must possess the skill, knowledge and ability essential to the successful performance of assigned duties


Preferred Qualifications:

Bilingual skills

Certification in Case Management

Knowledge of California Children's Services benefits authorization process (CCS)

Knowledge of University and medical center organizations, policies, procedures and forms

Nursing experience in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services

Previous experience working as a Case Manager
